AI alone won’t fix healthcare’s productivity problem

A new McKinsey analysis puts some hard numbers behind healthcare’s productivity challenge. Over the past 20 years, labor productivity at US clinical-care organizations has fallen ~1%, while productivity across the broader services economy increased more than 55%. That is despite healthcare organizations now spending over $150B a year on IT.

McKinsey argues the real gains will come from redesigning workflows around AI. A redesigned med-surg unit using virtual nurses, ambient documentation, and different staffing models could lower labor costs by more than 20%, with some systems seeing first-year RN turnover fall over 60%. On the back office, up to 50% of administrative work may be automatable, while agentic revenue-cycle workflows could improve productivity by as much as 40%.

That is an important distinction for health AI companies. Selling another point solution probably will not move the needle enough. The bigger opportunity is becoming part of an end-to-end workflow that actually changes staffing, costs, and how work gets done. The future of healthcare is patient to agent

Around 300M people ask health questions on ChatGPT each week, but most AI health interactions still end at advice. A patient can upload a lab result, understand what it means, and learn what test may come next. Then the patient still has to navigate the healthcare system themselves.

For a Healthcare AI Guy Media original, health AI founders Andrew Arruda and Josh Gray break that missing workflow into three pieces: Read, Admin, and Clinical. AI can already read records and explain results. Acting requires access to the patient’s full medical history, permission to work on their behalf, and connections into scheduling, benefits, prior auth, claims, and other administrative systems. Clinical decisions add another layer because a licensed clinician still needs to own actions like prescribing, ordering labs, and referrals.

The opportunity is connecting these pieces. If agents can securely access data, carry patient authorization, complete admin work, and hand clinical decisions to the right professional, healthcare AI moves from answering questions to actually helping patients complete their care.

The health AI earnings check-in

Startups are often the leading indicators of where health AI is heading, how quickly adoption is moving, and which business models are starting to work. But earnings season gives us something different: financial statements. Q2 results offered some clear signals.

  • AI-native companies are starting to show commercial scale. Tempus, which combines genomic testing, clinical data and AI software, generated $383M in revenue, up 22% YoY, and signed roughly $200M in new Data & Applications licenses. RadNet’s DeepHealth radiology AI and software business grew Digital Health revenue 57% YoY, while ARR nearly doubled to $106M.

  • Provider adoption is becoming measurable. Doximity said AI Search queries grew more than 25% QoQ, while SOPHiA GENETICS, an AI-powered genomics platform, saw analysis volume rise 22% YoY, including 60% growth in the U.S. and 70% growth in liquid biopsy.

  • AI-enabled healthcare platforms are scaling too, even if AI’s contribution is harder to isolate. Hinge grew revenue 53% YoY, Omada roughly 44%, and Clover 56%. AI is increasingly showing up across care navigation, clinical decision support, personalization and operations, but distribution, membership growth and better unit economics are still doing much of the heavy lifting. Hinge’s $105M acquisition of Cylinder also shows how stronger digital health platforms are using their core businesses to expand into adjacent conditions.

  • Healthcare infrastructure may be one of the quieter AI winners. Waystar, a healthcare payments platform, grew revenue 18% while continuing to roll AltitudeAI into claims and payment workflows. IQVIA, one of the largest data and technology providers to life sciences companies, grew nearly 9% and cited increasing adoption of its AI products.
