Healthcare AI needs a new payment model
A new Peterson Health Technology Institute report argues that today’s healthcare payment systems aren’t well suited for clinical AI, especially as tools move from supporting clinicians to taking on more of the work themselves in an agent-driven world.
The report uses hypertension to show where the current model starts to break. The condition has clear outcomes, established treatments, and fewer than 1 in 4 US adults with hypertension have it under control. AI could monitor blood pressure continuously and adjust medications between visits with far less clinician involvement.
Under fee-for-service, that creates the risk of more billable activity at very low marginal cost. Capitation and pay-for-performance fit better, though they often fail to cover upfront investment, workflow redesign, and the challenge of attributing savings to a specific tool.
The proposed answer is a deflationary model tied to measurable results, with prices falling as AI scales and operating costs decline. CMS’s 10-year ACCESS model offers an early example: 50% of payment is provided upfront and the rest depends on outcomes. The core idea is straightforward. Pay for better blood pressure control and lower total costs, rather than every visit or AI-generated action. As clinical AI becomes more autonomous, we’ll need to evolve reimbursement with it. AI is becoming healthcare’s first opinion
A new ZS survey of roughly 9,000 consumers and 1,400 physicians across the US, Germany, and China shows how quickly AI is changing where healthcare decisions begin.
In the US, 18% of consumers already use AI for health information, and 89% of them find it helpful. Another 42% research symptoms online before deciding whether to see a doctor, while 52% have requested a medication by name. Providers are seeing the shift too, with 68% reporting more patients asking for specific therapies.
A lot of this behavior seems driven by frustration with the system, which isn’t new news. 30% of US patients don’t have a primary care doctor, 42% wait more than 3 months to see a specialist, and 30% say diagnostic delays worsened their condition. There’s also a clear coordination gap: 83% of US doctors say they help coordinate across specialists, while only 58% of patients agree. These gaps are spaces where patients start self-navigating, and it's the doorway AI walked right through. (link)

What nurse practitioners teach us about clinical AI
Nurse practitioners may offer the most related roadmap for how autonomous clinical AI reaches the mainstream.
NPs started in the 1960s as a response to access shortages, then spent decades fighting for state practice authority, reimbursement, legal recognition, clinical evidence, and centralized advocacy. Today, there are 461,000 NPs and more than 200,000 PAs. Together, they represent more than 1/3 of US prescribing capacity and deliver as much as 25% of Medicare visits, up from 14% in 2013.
Clinical AI will likely follow a similar path. Use will move ahead of legal clarity, state rules will vary, court cases will define the boundaries, and CMS reimbursement will shape adoption -- all of which we’re already seeing. The hardest question may be liability: who is accountable when an AI system makes a clinical mistake?
The takeaway is that technology alone will not drive adoption. In healthcare, regulation, payment, evidence, and coordinated advocacy matter just as much. Fortunately, the groundwork is already being laid. (link)
